What are Care Management services?

Our dedicated Care Management team offers LGBTQ+ elders and caregivers access to LGBTQ+ affirming supports and programs across New York City. Elders rely on families and friends for critical support in times of need. But sometimes providing this kind of caregiving for a friend or loved one can feel overwhelming.

SAGEServes is here to help with:

  • Homecare respite
  • Social adult day respite
  • Supplemental funds for home modifications, Personal Emergency Response Systems, glasses, incontinence products, transportation expenses, and more
  • Caregiver support groups
  • Case management
  • Respite Buddies program
  • Short-term counseling

We also provide referrals to and recommendations for safe and LGBT-affirming long-term care and home-based care services.

Who is eligible to receive assistance?

Our Care Management team serves elders in the New York City area. They provide holistic coordinated care to LGBTQ+ elders and the people who support them. If you are an LGBTQ+ person age 60 or over (50 or over for those living with HIV/AIDS), SAGEServes can help you. If you are a friend, family member or partner—even if you are not LGBTQ+ or over 60—who has been caring for an LGBTQ+ older person, SAGEServes can help you as well.

How do I get help?

SAGEServes’ Care Management team members are located at our various SAGE Centers and national headquarters at 305 7th Avenue, NYC. SAGEServes Care Managers are available by appointment Monday through Friday. Please call 212-741-3590 to make an appointment.

Professional Opportunities

Are you a case worker, aging services provider, or social work professor or administrator who is interested in learning more about how to serve LGBTQ+ older adults?

SAGEServes Social Services offers training and technical assistance to LGBTQ+ and aging organizations throughout the New York City area, as well as training for interns at social work programs in area colleges and universities.
